Plan Activation Instructions

  1. Ensure that your full name, vehicle year/make/model, and start date are filled out completely on the back of your certificate.
  2. Read the Customer Guidelines and sign the back of your certificate to acknowledge and authorize that you fully understand the services and credits included on your maintenance plan.
  3. On or after the authorized start on the back of your certificate, complete the form below to activate your maintenance plan.
  4. Keep your maintenance plan in a safe place so that you can present it to the service center at the time of service.

    1. I acknowledge that I am the authorized owner of the two-year maintenance plan for which I am activating, and I shall present the plan certificate to the service advisor PRIOR to service at the time of each appointment.
    2. I understand the two-year maintenance plan is NOT A WARRANTY and does not include and/or cover repairs, services, or discounts other than what are explicitly printed on the plan certificate.
    3. I will request an estimate prior to service so I understand my financial responsibility to the service center. In the event my vehicle requires specialty or synthetic motor oil or supplies, or if the service center does not stock conventional motor oil, I understand that the service center is required to charge the additional balance due above and beyond a standard, conventional oil change (covered under this plan), and I will be responsible for any balance due to the service center.
